Job Statement

Under the directions of the Coding Services Supervisor and Manager, this position will be responsible for CPT and ICD-10 coding and ensuring accuracy and maximum reimbursement.

Job Duties

An effective Certified Professional Coder will exemplify the MediRevv Mindset by helping the organization on a whole achieve balance between partners, people, and performance through:

• Reviews and analyzes patient records according to current compliance policies and providers documents are compliant.
• Assigns accurately and sequences appropriately ICD-10 and CPT codes and all applicable modifiers

• Monitors regulatory and payer changes as they apply to diagnostic and procedure coding

Job Requirements – Knowledge, Skills and Abilities

A successful candidate must have proficient knowledge/capabilities in the following areas:

• Nationally recognized coding credential including, but not limited to CPC, COC, CCS, CCS-P, RHIA or RHIT through AHIMA/AAPC.
• High school diploma or equivalent required.
• 1-3+ years coding experience required, and outpatient physician and/or multi-specialty coding experience, preferred.
• Understanding of all or a combination of ICD-10, CPT, HCPCS, modifiers, medical terminology and HIPAA compliance.

Responsibilities:

• Review coding for accuracy and completeness prior to submission to billing system utilizing CCI edits. • Make forward progress within the training period toward meeting coding accuracy standards of 98% within the first year of employment. Meet appropriate coding productivity standards within the time frame established by management staff.
• Code all diagnoses and procedures by assigning and verifying the proper ICD-9-CM and CPT codes (DSM IV if applicable). Assign the principal and secondary diagnoses and procedures by thoroughly reviewing all documentation available at the time of coding.
• Utilize computer applications and resources essential to completing the coding process efficiently, such as hospital information systems (Medipac/SMS/Meditech), encoders and electronic medical record repositories. If applicable, abstract required medical and demographic information from the medical record and enter the data into the appropriate information system to ensure accuracy of the database. Correct any data to be in error after reviewing the medical record and comparing with system entries.
• Refer problem accounts to appropriate coding or management personnel for resolution
• Complete work assignments in a timely manner and understand the workflow of the department. Maintain daily productivity statistics and submit a weekly productivity sheet to management clearly indicating the number of hours worked, the number of coding hours, the number of average charts per hour, and number of minutes/hours spent on non-coding tasks.
• Determine diagnoses that were treated, monitored and evaluated and procedures done during the episode of care and assign appropriate codes. Review appropriate documents in the patients’ charts to accurately assign a diagnosis and/or procedure. Ensure the diagnoses and procedures are sequenced in order of their clinical significance to accurately assign the appropriate DRG/APC/ASC or payment tier under the Prospective Payment system or DSM IV methodology to guarantee accurate reimbursement on UPMC patients.
• Identify incomplete documentation in the medical record and formulate a physician query to obtain missing documentation and/or clarification to accurately complete the coding process. Consult with DRG Specialist when applicable during query process.

Qualifications

• High School or GED equivalent. Completed an AHIMA or AACP-certified Coding program, Bidwell Training School or equivalent program. Curriculum includes Anatomy and Physiology, Pharmacology, Pathophysiology, Medical Terminology, ICD-9-CM and CPT Coding Guidelines and Procedures.
• Outpatient: pharmacology is taught on the job during training; pathophysiology not required.
• Inpatient: Pharmacology & pathophysiology coursework required.

Basic Qualifications

This is a remote role. CircleLink Health is looking for passionate, tech savvy registered nurses to work remotely and serve patients enrolled in Medicare’s Chronic Care Management Program. In this part time role (20-25 hrs per week), an RN Care Coach will be assigned a group of patients that they will be following and calling each month. In these monthly calls you will provide education, coordinate care, close preventive care gaps, and coach on strategies for self-management to keep them out of the hospital.

Key Responsibilities:

• Utilize our specialized care management software to call Medicare patients with 2 or more chronic conditions (Diabetes, CHF, Chronic Pain, COPD, etc.) on a monthly basis
• Build and maintain rapport with patients to help coach them to improved health through SMART goals and education on self-management strategies
• Implement and improve the Plan of Care by updating medications, appointments due, biometrics, symptoms, and interventions made
• Connect the patient with community resources as needed, including transportation, personal care needs, prescription/DME assistance, social services, etc.
• Conduct Transitional Care Management activities to high risk patients discharged from the hospital and the ER to reduce unnecessary readmissions.
• Close care gaps by encouraging and assisting with preventive care measures, i.e. annual well visits, vaccines, cancer screens, follow-up/specialist appointments, etc.

Compensation:

A clinical encounter occurs after two criteria are met: a patient has a successful clinical call and the patient has 20 minutes or more of time in their chart timer.

• Ex: If in one hour you called and spoke 3 patients and spent 20 minutes with each of them, your pay for that hour would be $45.00 per hour ($15.00/pt reached x 3)
• Ex: If in one hour you called and spoke with 2 patients and spent 30 minutes with each of them, your pay for that hour would be $30.00 ($15.00/pt reached x 2).

Pay Timing:

Monthly via direct deposit, 40 days after the last day of the month of service. This is due to the time it takes Medicare to process reimbursements, but your monthly pay is guaranteed after the month is over.

A Telephonic Nurse Case Manager is responsible for providing telephonic medical case management for Worker’s Compensation and Liability Injury Management programs. Case Management is a collaborative process to promote quality cost effective outcomes, which enhance the physical, psycho-social, and vocational health of iniduals. This includes assessing, planning, implementing, coordinating, and evaluation of service options. The goal of the Case Manager is to assist the injured worker in receiving appropriate, cost effective medical care for their injury in a timely manner, and to expedite their return to work.

Responsibilities may include, but are not limited to:

• Once a file is identified as needing a Telephonic Case Manager, the TCM ensures telephonic case management activities are processed in accordance with EK’s standard. Activities may include:

o Completing referrals within computer database from either injury report or information gathered from the web-based system

o Performs 3-point contact on new files and, depending on account, forwards within 24 or 72 hours from date of referral

o Reviewing and evaluating all medical correspondence and provider reports obtained to determine work status of IWs or patients

o Reviewing treatment plan history and direction of file with client, when necessary

Essential Duties

Medical Billing

• Provide efficient and effective account receivable services on behalf of our member clients to maximize their reimbursement and support OCHIN revenue cycle performance indicators for financial health
• Accurately bill Medicare, Medicaid, self-pay/uninsured, and commercial insurance, processing claims in accordance with payer requirements and organization policy
• Assist with the collection of receivables by monitoring accounts receivables, checking claim status and resubmitting claims of overdue accounts, filing corrected claims or appeals and alerting supervisor of seriously overdue accounts and trends
• Post patient payments, electronic remits, and paper explanation of benefits (EOBs).
• Correct claim and charge errors

Qualifications

Included Health is looking for nurse practitioners/physician assistants who are passionate about caring for members holistically through their healthcare journey and ensuring needs are met with industry-leading interventions, within our Connected Care Program (CCP).

Nurse practitioners/physician assistants will guide members through complex medical situations, partnering with a multidisciplinary clinical team that includes a nurse care manager, other healthcare professionals, care coordinators, and records specialists, to deliver integrated remote care in an innovative way. The nurse practitioner/physician assistant should enjoy a team-based collaborative approach to member care including spending time on the phone with members, listening to members’ needs, answering questions, and serving as an advocate. They should also excel in creation of cohesive care plans and in providing clinical depth for a member’s care.

The nurse practitioner/physician assistant should possess the clinical acumen to help guide care for members and navigate available benefits and resources. The nurse practitioner/physician assistant will support members through complex care management, disease management, and acute case management, ensuring they receive longitudinal care that results in excellent health outcomes.

Responsibilities

• • Deliver coordinated and compassionate virtual Care Management by telephone and/or video that improves members’ health outcomes.
  • Generate impactful care plans together with members, nurse care managers and our multidisciplinary care team, and help members achieve the desired goals, in both acute and longitudinal settings.
  • Partner with the members’ local providers to ensure coordinated care. Coordinate necessary resources that holistically address members’ problems, whether clinical or social.
  • Provide clinical depth to a member’s care and help to identify opportunities to improve a member’s healthPartner with our Credentialing Team to become licensed in additional states if needed.
  • Model our culture and values through humility and curiosity.

Requirements

• • Active medical license and Board Certification required.
  • Willingness to become licensed in multiple states
  • Be comfortable guiding care for a variety of medical conditions and disease states.
  • Be highly empathetic. We work with members and their families who are going through challenging times. Ideal candidates practice empathy and reassure members that we are available to help them.
  • Ability to understand cultural and socioeconomic issues affecting members and to coordinate all available resources to serve members.
  • Collaborate well across multidisciplinary teams with clinical and non-clinical members to deliver a seamless, top-quality care experience to members.
  • Strong verbal and written communication skills. You will spend time on the phone with members and families, as well as a lot of time communicating with colleagues. Therefore, the ability to gather a clinical history, answer questions at a member level, and succinctly summarize findings is critical.
  • Must be able to work efficiently. We are a fast growing company and we are busy. Our team is expected to meet volume goals without sacrificing quality. Good judgment for balancing priorities is a must.
  • Be flexible and comfortable with working in a rapidly-changing environment. Be able and willing to work until 6pm local time, with occasional weekend commitments as well.
  • Strictly follow security and HIPAA regulations to protect our members’ medical information.
  • Strong competence and ability to use multiple computer/medical record systems. Experience with telehealth preferred.

Schedule

• • 6am-3pm PT OR 7am – 4pm PT, Monday – Friday
  • Occasional evenings (until 6p) and weekends

Primary Responsibilities:

• Reviews and resolves all services in assigned charge review, claim edit, and denial / follow – up work queues to ensure timely and accurate charge capture or insurance follow – up
• Accurately deciphers charge error reasons and plan follow – up steps
• Conducts charge reconciliation by reviewing various reports detailing billable activity and following-up, as necessary to ensure charges are submitted and cleared from missing charge reports
• Reviews all applicable data sources, including but not limited to, electronic health record, inpatient admit, discharge and transfer (ADT) reports, operative logs (aka Op Logs), nursing home visit documentation, procedure reports generated from non – electronic health record systems, etc.
• Documents clear and concise notes within the electronic health record according to established standards to facilitate comprehensive and accurate claim history
• Reviews electronic health record, scanning database and other systems to obtain required information such as referral / authorization numbers, Advanced Beneficiary Notice (ABN) or financial responsibility form (FRF) capture, accident or work injury dates, etc.
• Follows all department policies and procedures, desk level procedures, guidance documents, or other work tools designed to ensure accuracy
• Performs other duties as assigned
